Limit ILOHC to once per TCI / TCO contract

Currently it is possible to include ILOHC multiple times in the same TCI / TCO with no system warning in place. This is only clear in the SOA.
This ILOHC is only usually payable once per contract (unlike IHC) - There should be a configuration flag to set a limit of 1 ILOHC per TCI/TCO, or at least a warning pop up (similar to the offhire warning box).
