Enhancement to Voyage Instruction handling

We have created multiple Voyage Instruction forms that we use internally to collect data from Operators and Charterers collectively.
However, current system limits the functionality of what we are trying to achieve.
We need to be able to create multiple of the same form (e.g: 'hold failure' form - we could require multiple hold failure forms per voyage)
We also need the ability to Reject a Submitted / Approved form
